Congress created the 340B program to help safety-net providers serve low-income patients. But lawmakers never required hospitals to pass the savings directly to patients or clearly disclose where the money goes. That lack of accountability has helped turn 340B into a major source of hospital revenue. Some hospitals can mark up discounted drugs by 10 times or more, while patients still face high out-of-pocket costs.
